MFM Functional Patient
For all prospective new patients, we start with a Free Discovery Call. That's a 15 min call with our Doctor or Nurse Practitioner for us to discuss your health needs and issues and see if we can help you. For Functional Medicine Patients we then schedule you for a Free Consultation office visit with the Doctor or Nurse Practitioner. If you choose to go forward, we will start your functional medicine deep dive intake sequence. You will be asked to fill out a comprehensive intake package, a food log, a BM log and sleep logs. Phase I Intake will be 4 visits or more, some by phone. In Phase II Intake we will perform your MFM Physical Functional Examination. In Phase III we will give you a full written Report of Findings and a cost estimate gonging forward. Phase IV is your treatment.

The first step to become a Functional Patient is the Free Consultation office visit The Consultation Office Visit is just that. It's a 30 min meeting with the Doctor or Nurse Practitioner for you to tell us what's going on with you and for us to listen to you and see if we can help you.

Based on your medical problems we'll decide if you're Functional Medicine Fast Track or Functional Medicine Full Track patient. This decision is not based upon the complexity of your problems but based on the number of your problems. One just takes longer to deep dive due to the number of problems.
** We charge a $30 scheduling fee for the Free Consultation. If you decide to become a patient, we give you credit for the $30. If you don't become a patient, we refund your $30 on the spot. If you don't show for your free consultation you forfeit the $30.
We block 30 minutes in our schedule for your appointment; and, if you don't show then others can not get in to see the doctor.

Functional Medicine Phase 1 is called - The Intake History. We charge a single flat fee for Phase 1 - Intake History no matter how long or how many visits it takes. For Fast Track patients we normally deep dive into 1-3 major problems and 1 minor problem. For
Full Track patients we normally deep dive into up to 6 major problems an up to 3
Functional Medicine Phase 1 is called - The Intake History. We charge a single flat fee for Phase 1 - Intake History no matter how long or how many visits it takes. For Fast Track patients we normally deep dive into 1-3 major problems and 1 minor problem. For
Full Track patients we normally deep dive into up to 6 major problems an up to 3 minor problems. This can take 4 or more weeks of intensive history work by our team of RN's. Both tracks includes several hundred dollar of basic intake testing. 1) Basic Bloodwork (unless you had some in the past 3 months) 2) Urinalysis, 3) Basic Minerals Testing 4) Basic Toxic Testing and 5) Basic antioxidant testing.
We're going to ask you to keep Food logs and BM logs and Sleep logs. During this time, we also get and review your past medical records, past test and past imaging. We may need to order new or additional blood work or other essential test during your intake.

Next comes Phase 2 - The Functional Physical Examination visit. We charge a flat fee for the Functional Physical Examination Visit.

The Doctor or Nurse Practitioner will do a fully clothed Functional Physical Examination with a nurse in the room with us at all times. We do not examining male or female genitalia. Our patients tell us its the most comprehensive physical examination they have ever had.
If during the functional physical examination we find that we need to do a color doppler ultrasound, then the cost for the ultrasound will be a flat fee of $150 more. Sometimes you just need to see organs or other things more clearly.

We know you want, NO SUPRISES. At this visit the Doctor or Nurse Practitioner will give you three documents: #1) A Differential Diagnosis {a list of what we found and our reasoning} #2) A Millar Functional Medicine Treatment Plan [just how are we going to treat all the things on your differential diagnosis list, that are treatable] and #3) A No Suppresses Act estimate of cost laying out additional test needed and their cost. What treatment you need and the cost. And what nutraceuticals, supplements, IV;s, Homeopathic Remedies or Peptides are needed and the cost.
Some patients want to treat everything on the treatment plan list others a few things now and a few things later. Remember we work for you, so just tell us what you want to treat.
Phase 4 - Functional Medicine Treatment Plan
Phase 4 is setting up a plan to treat what you want treated. Its also figuring out how to pay for your treatment. Again some want to treat all their treatable problems while other just want to treat some of them. So Tell us what you want to treat now and we'll create a payment plan just for you. We offer a 15% bookkeeping discount for paying in full in advance.
Now you have two choices. You can pay for visits one at a time. OR you can purchase a package at a substantial savings. We offer convenient no interest payment plans. For payment one visit at a time, our Providers are just like other professionals, lawyers or accountants. We charge an hourly rate. So if you have a 30 min appointment then you pay for 30 min of time. Just like lawyers or accountants we also charge for time we spend on your case when your not there or for phone calls. Just like a lawyer we ask for a retainer fee.
However if you purchase a package then its all included. Your package covers face to face meetings, phone calls, and time we spend on your case when your not there.
Our Business Manger will go over payment options and set up a payment plan if needed. Once we have your approval to go forward with treatment, we start.
