Welcome to Millar Functional Medicine FAQ

Most health insurance does not cover or pay for functional medicine. We do not accept any health insurance. All patients are self-pay by private contract between the patient and the provider. We are not in network, not out of network and not providers for any insurance companies. If you have a HSA, FSA or HRA your tests may be covered. That is between you and your HSA, FSA or HRA plan administrator.  

The basic answer is No. BCBS does not pay for functional medicine services. Patients with BCBS are self-pay by private contract. BCBS may pay for some of the blood, urine and stool testing in rare instances. We are not in network with BCBS but can occasionally order some limited tests from an in network BCBS lab. Many patients however find it cheaper and less of a problem to just pay our cash price for tests. 

Again Medicare does not cover functional medicine. All Medicare patients are self-pay and must sign a self-pay agreement as a private contract between you, Millar Functional Medicine and its providers.   

Again the VA or Tricare do not cover functional medicine. Patients with VA or Tricare must sign a self-pay agreement.  

We offer all prospective new patients a Free Discovery Call, a 15 minute telephone call with the doctor or nurse practitioner to discuss your problems, wants and needs. After the Free Discovery Call, we will decide if you're a therapy patient or a functional medicine patient. if your therapy patient we'll set your intake appointment OR for functional medicine patients ,more difficult cases, we'll set you up for a 30 min Free Consultation** appointment meeting to further discuss your problems. 

** We charge a $30 scheduling fee for the Free Consultation. If you decide to become a patient, we give you credit for the $30. If you don't become a patient, we refund your $30 on the spot after your 

If you decide to go forward then we start the intake process. The intake cost $350 and covers 3-4 intake office visits. We will ask you to fill out our extensive 45+ page intake questionnaire as well as keep a log of food eaten, your stool and your sleep. This includes some intake testing like Urinalysis, Toxicity test, and Minerals test. Most people apply their $30 scheduling fee to this intake fee for a net due of $330. Next comes the MFM Functional Physical Exam. 

Next comes the The MFM Functional Physical Examination. The cost of Exam is $250. We'll do a fully clothed functional physical medical exam. Most of our patients tell us that our exam is the most thorough exam they have ever had in their lifetime. Next we will prepare a detailed written report of our findings. 

We charge a flat fee of $250 for the Report of Findings visit. We give you a detailed report of what we found, a list of your diagnosis and our functional treatment plan. We will also give you a disclosure of what tests are needed and the cost, what treatment we recommend and the cost,  and what supplements, IV's,  injections or other treatment we think you need and the cost. We will then together decided how fast you want to treat your problem(s) based upon your budget.  

For most patients after they have paid for and completed the intake sequence they can expect an out of pocket cost of between $2500 to $3000 for a single problem. We offer no interest written payment plans for all treatment programs. We offer a bookkeeping discount of -15% for those that pay for their treatment up front. We also have a one year Optimal Health VIP program available where we include all of our treatment programs. and VIP access to the doctor.
